RefyllRx is designed for Ontario operations. Personal health information submitted through an assessment is collected only to coordinate and support professional review and follow-up.

Information we collect

Depending on the assessment, information may include your name, contact information, age range, current province, reason for review, symptoms, general medical history, regular medicines, allergies, previous care, and information you choose to add.

Why we collect it

We use assessment information to organize the request for a regulated healthcare professional, support safe follow-up, and maintain records associated with the request. The website itself does not diagnose a condition or make a treatment decision.

Knowledgeable consent

Before an assessment can be submitted, you are told why the information is being collected and asked to provide consent. You can choose not to submit the form. A healthcare professional must separately confirm your identity and current location before providing virtual care.

Storage and access

Assessment responses are transmitted to the RefyllRx backend and stored with restricted database permissions. Public website visitors cannot read assessment records. Access should be limited to authorized personnel who need the information for the purpose for which it was collected.

Email notifications

Notification emails contain a reference number and care area only. Full clinical questionnaire answers are not placed in ordinary notification email. The secure backend remains the source of the assessment record.

Safeguards

RefyllRx uses server-side validation, restricted database permissions, anti-spam controls, transport security, and data-minimization practices intended to reduce unauthorized access, use, disclosure, copying, modification, or loss. Operational safeguards, access controls, audit practices, vendor agreements, retention rules, and breach-response procedures must also be maintained by the organization operating the service.

Access, correction and privacy questions

You may contact info@refyllrx.com to ask about information you submitted or to request access or correction where applicable. Do not send detailed personal health information by ordinary email.

Ontario scope

This website is currently designed for Ontario care pathways. Canadian privacy and health-profession requirements vary by province and territory. Expansion outside Ontario requires a separate jurisdictional review.

Emergencies

Website forms are not monitored for emergencies. Call 911 or seek urgent medical care for severe or rapidly worsening symptoms.
